Description. Contractor will provide service, labor, materials,and supervision to control all insects for the interior and exterior of all buildings and property owned or leased by the VA Medical Center, 800 Zorn Avenue, Louisville, KY 40206. The contract will be for a base year and four one-year options to renew. The solicitation will be issued o/a September 9, 2005. All questions must be in writing and may be faxed to 615/ 867-5429 or emailed to john.gregorio@med.va.gov. No telephone calls will be accepted or returned. This procurement is a Total Small Business Set-Aside for a commercial items contract. The following clauses and provisions plus addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes and Executive Orders. The offerors shall register with the Central Contractor Registry and the Online Representations and Certifications Application (ORCA). Instructions are included in the solicitation.
