From a program that started in 2014 with one visit to Tucson Medical Center for Children - thanks to Truly Nolen's then-director of philanthropy Michelle Nolen - her personal vision grew to five hospitals in three states in 2015, and included a then-record 47 hospitals last year.

This year, Truly Nolen visited a record 58 hospitals, child care facilities, domestic violence shelters, and development disability facilities across 13 states and four Canadian provinces as part of the company's "Truly Spooktacular" program.

The program, which provides children who may not be able to go trick or treating for Halloween the opportunity to decorate plastic pumpkins with sticker kits during their hospital stay, included visits throughout Arizona, California, Florida, Georgia, Kentucky, Nevada, New Jersey, New Mexico, North Carolina, Nova Scotia, Ohio, Ontario, Prince Edward Island, Saskatchewan, South Carolina, Tennessee, and Texas.

In addition to TMC for Children, three other hospitals have been affiliated with the program since its second year in 2015. These additional legacy hospitals are AdventHealth Orlando (Orlando, Fla.), Palm Beach Children's Hospital (West Palm Beach, Fla.), and St. Joseph's Children's Hospital (Tampa, Fla.)

First-time participants in 2025 included Arnold Palmer Children's Hospital (Orlando, Fla.), Beyond 21 Hub Foundation (Cornwall, Ontario), Cincinnati Children's Hospital Medical Center (Cincinnati, Ohio), Cleveland Clinic Tradition Hospital (Port Saint Lucie, Fla.), Cochester East Hants Health Center (Truro, Nova Scotia), Johns Hopkins All Children's Outpatient Clinic, (North Port, Fla, and Sarasota, Fla.), Prince County Hospital (Summerside, Prince Edward Island), Queen Elizabeth Hospital (Charlottetown, Prince Edward Island), University of Florida Health Shands Children's Hospital (Gainesville, Fla.), and Wolfson Children's Hospital (Jacksonville, Fla.).
